As a Payroll Application Service Provider, Crest Payroll collects personal identifiable information about Payroll Service bureaus, Companies and respective Employees. This information includes Employee Code, Name, Address, Birth Date, Telephone, Fax, Email, Tax Identification Number, Social Security Number, Salary Information of individuals, Statutory Information for companies and individuals that comply with federal and state laws, Banking Information. We also collect demographic information such as your ethnicity and religion. We will collect your legal status, start date, employee status, probation end date and whether or not you are an active employee. We will collect your Gross Wage YTD, Federal Income Tax deductions, Social Security Tax deductions, Medicare Tax deductions, and Earn Income Credit YTD. Generally, this information is requested when your profile is being set up in the Crest Payroll website as a user (role-based site access) and/or to perform relevant 'payroll processing' tasks for you based on your employment and residential information. We may use your Personal Information for primary purposes such as:

We may share personal information about you with banking institutions, who help us perform certain tasks related to payroll.
